Take the Vermont H1N1 Flu Response Survey

More than 650 Vermonters have responded to the H1N1 Flu Response Survey, and it is not too late to offer your opinion. Take 15-20 minutes to take the confidential survey and provide your perspective and insight into the state’s actions in responding to the flu.

We will not ask your name, and your responses will not be linked to identifying information.

All responses will be reported only in groups that will ensure confidentiality. Data collected from the survey will be included in an after-action report and improvement plan to strengthen the state’s capacity, capabilities and coordination in dealing with flu and other public health events and emergencies.

Thank you for your assistance with this important assessment effort. There are only a few more days left to respond to the survey so please fill out the survey as soon as you can.
